Heel spur and now a swollen ankle

I have had a problem with a heel spur which orthotics have not seemed to help – for about six years. I have suddenly for the first time noticed a large amount of swelling in the ankle of the same foot along with a lot of generalised pain in my foot all the way up past my knee. Any ideas? Could I have a fracture?
