?piriformis syndrome

I am a 31y.o female with a 12 month Hx of lower back pain,mainly being compression.This originated with severe bilateral knee pain,followed by extreme hip painwhich then radiated into mid gluteal region.This then extended up into my lower back.A CT showed a minor discbulge at L5/S1 and a recent MRI revealed no problem.Ihave had physio,chiro,deep water therapy,orthotics and gluteal and itb stretching.I have recently been diagnosed with patello-femoral arthritis.I now have severe LBP and nerve pain in both legs to foot.HELP?
