A painfull torn shoulder does not cause a heart attack. Unfortunately your grandmother has probably had heart disease for quite some time now, although she may have not been aware of it. As far as treatment for the torn muscle, she can safely do gentle stretches if it’s a torn pectoral muscle. If it’s a torn rotator cuff, she may have certian restrictions for movement. Ice the shoulder daily for about 15 mins.
