Rotator Cuff Problems

I’m having similar shoulder problems on both sides, by far worse on my right and dominant side. During abduction of the right arm, after raising only 45 degrees from hands down, I begin to feel like my elbow joint is being pulled out of place, the further up or up and back I attempt to raise my arm (either extended or tucked in, similar effect), the more impossible it is due to a combination of pain and flat out lack of range of motion.
My chest begins to cave in and in order to continue lifting at all my trapezius has to be completely elevated, and I have to really put effort into it.
My left side is hinting at the same problem in time, as I feel similar sensation in my left elbow during abduction, just at a degree from hands pointed down.
Transverse abduction isn’t like this however, still a bit weaker on the right side, but much more range of motion.
I don’t think my suprispinatus is too much to blame as when I raise my hand overhead in front or just straight up, I’m able to relatively ok.
It’s only on the sideways abduction and even more particularly as I try to move my humerus backwards a little during abduction, it then becomes even more impossible, causing great elbow discomfort (like it’s going to pop).My infraspinatus is definitely a problem as it’s very low in feeling from the lack of circulation and high in pain at trigger points, and the same with teres minor.My trapezius and deltoids are ultimately getting way overworked now in assisting with these limited ranges of motions.
Where should I attack?
What’s causing these sorts of limits/weaknesses? Any more information that may help you give me help is readily available.

    External Rotation is extremely limited as well, particularly on the right side, so should I assume this is due to subscapilarus tightness not allowing the shoulder blade to rotate out with infraspinatus contraction?
