Top of foot pain

35 year old runner. 20 plus years of distance running. 40-50 miles weekly. 8 daily average. 6’4 200lb male. over the past year severe burning in both feet. only on top. only hurt when shoes are on and when sitting. Positive tinnel sign on both feet at tibial ankle. mri did show bilateral foraminal stenosis L5-S1. with collapsed L5-S1. No low back pain. Severe middle back pain.
