archilles tendon problem ongoing!!!

Hi Had an MRI a few months ago for archilles tendon to check if it was fraying or rupturing slowly. Have acute tenditis. Have swelling of the left foot by the archilles tendon and ankle. Pain is shooting off and on. Very Tender to the touch wakes me up at night?Anti-inflammatory are doing a thing!! Even had the comfort boot. Any suggestions on what else to do, perhaps orthotics or cortisone injections? Thank-you
