mid back pain for over a year

About 1 1/4 years ago I lifted my desk at work in avery stupid manner,i felt a twinge in my mid back then but figured it would go away. I first noticed an area to the right of my spine felt like it was numb or dead, then one day I had a sensation of cold ice water being poured down the inside of my right leg. thats when I went to a doctor. I’ve been given-mobic/esgic/celebrex/lodine/flexiril,cortizone shots(2),hydrocodone apap. the only combination that makes me pain free is the hydrocodone(2in morning 1 at lunch 2 in evening). I can’t sleep at night, I dread getting up because I know i’ll hurt all day long, its hard to concentrate on work when im hurting, and im very irritable with my family after I have hurt all day long, I don’t want to have family time I just want to lay down for some releif. The hydrocodone makes it to where I don’t hurt, the lodine makes life bearable.Over the past month the pain has started to radiate down into my arms and hands. I have no insurance and must pay everything out of pocket. What should I do?How forceful should I be in relating what medications work? When I have called in to ask for refills they act like im sub human for needing anything to be able to have a normal life.
