The findings were: …and osteochrondal lesion of the superomedial aspect of the talar dome. A 3mm x 4mm diameter subcortical cyst is surrounded bt a moderate zone of T2 hyper-intense oedema.
There is mild irregularity of the overlying cartilage with very mild cartilage thinning but no fissuring nor full thickness cartilage defect.
Marrow signal intensity elsewhaere is normal.
There is no abnormal accumulation of fluid within the ankle joint and no thickening of the anterior synovium to suggest synovitis.
The subtalar and talonavicular joints appear normal.
The collateral ligaments and the tendons around the ankle appear normal. Conclusion.
Osteochrondral lesion of the talar dome, with changes in the bone more dramatic than in the articular cartilage.
No evidence of synovitis.My profile.
Male 48yo. Height 5′ 8″, weight 80 kgs, Former infantry soldier (so 15 yrs of marching, running with weight, parachuting, etc), very active sportsman, last five yrs active triathlete, 2015 half ironman.
Intending to do full ironman April this year.My questions are: is this an injury built up over time (overuse)? Is there treatment other than rest?
Will continued training worsen the condition? What might a long term prognosis be? Many thanks. Mark Cowan-Aston

    I do not know the specific cause of the lesion, but trauma is the usual suspect. Rest may or may not help, but I do not expect rest would cause this to heal totally. However I could be wrong. Patients continually amaze me with their wide range of tolerances for pain.An ankle brace may or may not help. You really should discuss this with your doctor.
