Pain just behind big toe

I have recently experienced intense pain on a bunion like lump just behind my big toe. The pain is on the top on the foot rather than the side which seems to be the norm with bunions. Also seems to come in pulses of often very intense pain. In general it is very slightly tender to touch but not constant pain. I’ve tried rest and ice and anti-inflamatory gel but to little effect. There are days I can run and feel no ill effects and other days when the pain makes it difficult to walk let alone run. I’ve had a bunion-type lump for year and apart from occasional aches if I’ve worn tight shoes it has never been a problem. Any suggestions?
