Post Tib Tedonitis
I have totally flat feet and have had lower leg pain for years.
Recently, the pain is focused around my right ankle, centered on and just behind the ankle bone on the medial side.
Could this be a manifestation of post tib tedonitis (most descriptions of post tib tendonitis suggest pain in the arches instead).

Yes that might be one possibility, but you really need an analysis of your gait and if needed an MRI to see if there is anything else going on.
