right ankle pain

i recently had an MRI of my right ankle.
Findings-moderately large joint effusion of ankle joint.
Abnormal signal intensity within subcortical bone marrowmof the distal tibia.
joint effusion of the subtalar joint.
fluid,within interosseous canal.
interosseous talacalcaneal ligament intact but fluid seen posterior to it and posterior to the posterior subtalar joint.
Abnormal signal intensity in subcortical bone marrow of the calcaneous.
no ligament or tendon injuries present.
have some degenerative arthritic changes.
have had increasing sharp pain over the last 3 months.
hard to walk especially up and down stairs. should I be in a cast to rest my ankle or go for physical therapy.
i ice it as much as I can.
i’m a teacher on my feet all day.
help please

    I fractured my right ankle 1999 and have had two sugeries since. In 2011 and another in 2013.I still have pain when standing and walking. My Ortho surgeon now wants to fuse the ankle to give me pain relief. I’m unable to stand for more than a few minutes let alone as you do all day.
