Ruptured Bicep Healing

I recently had surgery to repair a ruptured bicep.
It has been six weeks and the pain has not abated. In addition, I have a lump or knot about 3 inches above my elbow on the outside of the arm.
In addition, the pain is so great that it wakes me up at night. Does this indicate a problem with the healing?
What are possible causes? Could there be an infection/absess? Thank you.
