Shoulder pain

I am a college pitcher and I have been having front and toward the top shoulder pain, that seems to be inbetween where the arm and shoulder meet.
It is just a small area, and it does not hurt when I throw, but there is a little pain that flares up every now and then.
I was wondering whether it was tendonitis a tear or what… I have done very many light weight rotator cuff injuries and it has gotten better over time, and I do ice it, but I was wondering if anyone had any suggestion or ideas of what it might be.

    Hi I had injured my LT shoulder last July, wich resulted in a type 1 ac seperation, and it has flared up now for 2 months and the pain is great, and the Dr. said phyisio would not help, and that I will probably need surery, I really do not want to have surgery, be unable to go to the gym again for 6 weeks, is there andything I can do, maybe certain excersise to help heal the ac. if u can help I would be ever thankful, I can not afford physio. Thank You Chris
