Husband’s Elbow Surgery

Hello:My husband has had two tennis elbow surgeries in the past year on the same elbow. The last was Oct., 2012. He has been going to physical therapy since the surgery. He has a one-pound weight limit. The pain has not gotten better and if he shakes his arm, there is a definite “knocking” sound. The doctor is stumped. He is referring it to another surgeon.Any ideas on what causes the knocking and how it can be helped???
