Swelling behind knee

I have been running for years and have always had some knee problems.
The pain has increased since last January when I was training for a marathon and I have had significant swelling directly behind my knee for several months.
I have not been running for the past few months and have been cycling instead, but the pain and swelling continue. Do you have any idea what this could be and if it could be serious?

can u please give me some differential diagnosis on these symptoms.whole left side sever pinching painswelling at the neck, behind the both knees.left breast pain.the whole left side pain has been there for almost 20 years now but swelling just happen few months ago.
