Sesamoid fracture
What is the optimal timing for treatment of a sesamoid fracture?
If diagnosed immediatly do I have a better chance of a positive ( pain free) outcome. Mine was diagnosed 3 months after the injury and my podiatrist said if I had come in when I first injured it and had xrays and was diagnosed, early immobilization would have improved outcome and lessened the need for surgery

That certainly is true of any fracture. Surgery for the fractured sesamoid may be indicated if conservative measures such as padding, injections of cortisone, anti-inflammatory medication, icing and even immobilization fail.
